The following information is required for registration:
Company name and address of the contribution debtor and his legal representative,
current address of each place of business
the last address of the debtor reported to the state broadcasting organization
full name of the owner of the permanent establishment,
number of employees of the establishment,
contribution number
Date of commencement of the holding of the permanent establishment or of the motor vehicle liable to pay the contribution,
information on the branch or establishment affiliation
Number of contributing hotels and guest rooms and vacation homes
Number and place of registration of contributing motor vehicles
